If you have answered NO honestly to the questions in the form you can be reasonably sure that you are safe to exercise and therefore will be able to fully participate in your chosen activity. If you have any concerns please speak to your instructor and work within your limits.      

If you have answered YES to one or more of the questions in the form talk to your instructor about these points. Your instructor may restrict your physical involvement in this activity and offer you advice on these restrictions. Depending on the severity of these positive answers your instructor may ask you to refrain from the activity  before seeking medical advice from your GP. You will need to advise your GP of the answers that you answered positively to then medical approval to exercise will need to be obtained before full physical commencement of the course.

Has your doctor ever said that you have a heart condition and reccomended only medically supervised activity?
Do you have chest pain bought on by physical activty?
Do you tend to lose consciousness or fall over as a result of dizziness?
Do you have bone or joint problems that could be aggravated by the proposed physical activity?
Have you developed chest pain in the last month?
Are you on any medication?
Are you currently or have you been pregnant in the last six months?
Have you had surgery in the last 6 weeks?
Are you aware from your own experience or from your doctors advice of any physical reason why you should not exercise wih out medical supervision?
If you have entered yes to any of the above questions please state yes or no to confirm you have been approved by a medical proffesional to excercise?
